South County Civic Center, 16700 South Jog Road, Delray Beach.
It will be a drive-through.
You MUST have an appointment.
Appointments can be made starting at 10:00am by calling 561-804-0250.
(While this site was opened primarily to test older residents, aged 65+, anyone may call for pre-screening and appointments).
